"A sham doctor" is a logically reasoning in the dialogue. (A is B's friend who forced B to taking medication) Maybe changing and extending the meaning of จำเป็น flexibly from the right context or dialogue is needed. Anyway, thank you for your explanation. In lack of finding a better word in English I used fake / sham, but it doesn't mean it in a bad way. In a completely negative way I would use ปลอม instead of จำเป็น.
One example could be there is no doctor around, so you step in, you are a หมอจำเป็น <-- a doctor out of necessity.
Adding จำเป็น to the end basically means you are not actually in this profession, but you are doing it anyway due to whatever reason... it could be someone forcing you etc. etc.
